Fix and refactor Swagger unit tests

Swagger unit tests in org.opendaylight.controller.sal.rest.doc.impl package are using incorrect equals call on module revision when looking for desired module in EffectiveModelContext.

This in result means that assertions in tests are skipped.

Fix looking for desired module by namespace and revision, fix incorrect tests assertions, and refactor Swagger unit tests.
