Improve YangInstanceIdentifier.toString()

Our implmentation of toString() results in a lot of repetition of namespaces, which makes reasoning about the identifiers pretty much impossible.

As it turns out, we can simplify this greatly if retain the last emitted namespace and not emit it when not needed.
