[YANGTOOLS-635] Add context to exceptions thrown by yang statement parser

Not every parser exception contains the context (i.e. name+revision of a model that caused the error). Adding context to all parser exceptions would allow netconf (or any other layer on top of parser) to make their own decision for how to handle “flawed models” without forcing workarounds into YANGTOOLS.
