Allow codecs to be instantiated without a SchemaPath

Current API design, which requires a SchemaContext and a SchemaPath is not completely sufficient to disambiguate references and furthermore forces a lookup which can be avoided if the caller has the matching SchemaNode available.

Make sure JSONNormalizedNodeStreamWriter and XMLStreamNormalizedNodeStreamWriter can be instantiated by providing a combination of SchemaContext and DataNodeContainer.
